Community Behavioral Health Center (CBHC), a leading community mental health provider in northeast Ohio, is seeking qualified social workers (LSW, LISW) to join our team. The Counselor will conduct screening, assessment, treatment planning and provide psychosocial counseling to adult clients. In collaboration with our physicians and case managers, you will treat adult clients who have acute and chronic mental health conditions. This is an exciting opportunity to work in an outpatient setting as part of a multi-disciplinary treatment team.

The Counselor will conduct screening, assessment, treatment planning and provide psychosocial counseling and practical services to adult clients.
Identify the unique needs of clients and develop an individual care plan
Provide initial and ongoing clinical assessments, counseling, advocacy, and practical advice for clients and their families.
Document care activities and client progress in a timely manner
Demonstrate ability to work with diverse social, racial and ethnic groups
Master’s Degree in Social Work (MSW/MSSA) from an accredited school
Basic computer skills and experience with EMR preferred
Commitment to community-based health care services
Clinical Supervision provided, if needed
